Forward.Vision Export Requirements

U.S. Export Regulations require an export license before Forward. Vision.® Enhanced Vision Systems are sold and released to non U.S. Nationals in the U.S. or shipped to users outside the territory of the U.S.. Canadian sales do not require an export license if the sale is to a Canadian national. However, special export documentation and shipping is required.

In order to process an export sale, we require the following completed information and documentation along with an international money order, credit card or monetary wire transfer. Please click on the links at the bottom of the page to download and print the forms:

BIS Form 711 Explanation: The Ultimate Consignee is the entity to whom the product is shipped. The Ultimate Consignee must complete Blocks 1 thru 6 and sign Block 6.The Purchaser (entity signing the Purchase Order) must complete and sign Block 7. End User Agreement/Export Questionnaire Explanation: The End User can be the Ultimate Consignee, Purchaser or Third Party. If the aircraft is leased, the End User is considered the pilot(s) of the aircraft and the owner of the aircraft, since the aircraft reverts back to the owner at the expiration of the lease. Please complete each form with as much detail as possible including business and personal address, telephone numbers and postal codes. Proof of citizenship (copy of passport, if available) and certified copy of aircraft registration is also required.

To initiate the export licensing process, the completed forms can be faxed to 814.757.5631. Completed forms with original signatures and full payment must be received by Forward.Vision.® before shipment can be made. Export license processing usually requires approximately 6 weeks after receipt of all completed information and documentation.

In the event that an export license cannot be obtained, all deposits and fees will be refunded.
